Thank you for your interest in preparing a School Partner Agreement (SPA) with the School District of Philadelphia. This form will ask you to provide all of the information that is required to fill in the scope of work for your SPA.

Almost all of the information is required, so please take a minute to review what is required ahead of starting the form. Once you have the necessary information, you should be able to complete and submit this form quickly.

Supporting Documents:

If available, please submit your 1) logic model and 2) an evaluation report on your program conducted by a third party evaluator. If you do not have a third party evaluation, please submit details of the theoretical framework supporting your program’s approach and/or programming.
